SUDI Prevention Grant – Terms and Conditions

1. Purpose

The SUDI Prevention Grant supports community groups, organisations, and whānau champions who are passionate about raising awareness and supporting whānau with safe sleep practices.

All funded initiatives must align with the following key messages:

  • Face up, Face clear: Always place pēpi on their back to sleep with no pillows, toys or loose blankets.
  • Firm and Flat: Pēpi need their own flat, firm bed like a wahakura, baby-pod, bassinette or cot.
  • Free: Keep pēpi, especially their sleep space free from smoke, vape, alcohol and drugs. Free from harm at all times. Breastfeeding and immunising helps keep pēpi free from illness.
  • Fathers, Partners, Family: All members of the whānau play a role in keeping pēpi safe during sleep. Safe sleep is a shared responsibility that involves whānau and carers supporting pēpi to sleep safely. It’s important to know that SUDI prevention starts before birth.

2. Who Can Apply

Any legal entity, trust, or organisation is eligible for funding.

If you don’t qualify directly, you can apply in partnership with an eligible organisation.

3. What Can Be Funded

Funding (up to $5,000) can go towards:

  • Venue or equipment hire
  • Volunteer support
  • Equipment for activities
  • Promotion of the activity
  • Medals, prizes, and giveaways

4. What Won’t Be Funded

Funding cannot be used for:

  • Salaries, wages, or admin costs
  • Capital costs (e.g., facility development)
  • Costs that generate future income

5. How to Apply

Check you meet the eligibility criteria.

Gather your documents (proposal, budget plan, etc.).

Complete and submit the online application.

6. Approval Process

Applications are reviewed by the SUDI Prevention Coordination Service.

You’ll receive a response within 10 business days.

If approved, you’ll sign a funding agreement before funds are released.

7. Accountability and Reporting

Recipients must:

  • Use the funding exclusively for the approved kaupapa
  • Keep accurate financial records
  • Submit a final report, including:
    • Attendee numbers
    • Photos (with whānau consent)
    • Feedback and key learnings
  • Inform us as soon as possible if there are any changes to your planned event or activities

8. Misuse of Funds & Repayment

Funding may be revoked if:

  • It’s used for unapproved purposes.
  • Reporting requirements aren’t met.
  • The project doesn’t align with agreed objectives.

If funding is misused, repayment may be required.

9. Changes to Terms

The fund provider reserves the right to update these terms with notice to recipients.
